Job Summary for Masters Level Therapist:
The Masters Level Therapist is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of health care services. The Masters Level Therapist is responsible for supporting the healthcare team towards a smooth transition from one level of care to another in support of the patient/family.
This position provides the opportunity to serve an established panel of patients in a medically underserved community. The provider will have the support of in-house wrap-around services including Outreach Specialists, Care Managers, and Patient Care Coordinators. JPCHC is a collaborative office known as a pillar of care in Shelby County.
Experience:
Master's Degree in Social Work required.
LCSW, LCAC, LMHC, or LMFT licensure required.
CPI (Non-Violent Crisis Intervention) certification preferred.
Requires relevant health care, mental health or community-based experience.
Requires knowledge of the impact of the disease process and of human behavior to provide psychosocial assessments, short-term therapeutic counseling, and crisis intervention to patients, families and staff.
Requires knowledge of state and county adult and child protective legal mandates.
Requires knowledge of the principles of growth and development over the life span.
Maintains principles, practices and techniques of crisis intervention and de-escalation techniques.
Develop and maintain strong working relationships with the community, medical teams, law enforcement, adult and child protective services.
Experience collaborating with physicians, NP, etc. as part of interdisciplinary team
